State/Province/Region
Parent category link (e.g., "Wikipedians in <parent country>")
Name of the parent country (this will be used as the sort key within the parent category)
Optional Wikitext appended after the link to The State/Province/Region, so you can add regional noticeboard links, etc. It defaults to a period.

To create a category for wikipedians in Massachusetts in the United States:

Note that in the below example output, the "To add yourself to this list..." segment is incorrect because it uses the {{PAGENAME}} variable. When you place this on the category page, it will work correctly.
